1. Send your completed application and Code of Ethics forms to Barb Levenson or Patti Engle. Please indicate if you purchased the membership via Paypal if a check is not enclosed. Renewals may skip this step unless you care to update information or submitting payment by check. The link below downloads the .PDF into your computer's Downloads folder.

2. Select one:

Annual Membership - $30.00

Sept 1 through Dec 31 (current year) - $15.00

Payment by check is preferred. It greatly helps reduce expenses paid to third party payment apps. Simply include it with your application.
